Please provide your signature at the end of this document to acknowledge and accept our MJMS camp policy/waiver. A waiver must be signed and submitted for each camp participant for each camp week. 

1. Medical

I/we authorize MJMS or an individual associated with MJMS, in the event of an emergency, to use reasonable discretion, on behalf of myself/us and my/our child(ren), in delivering first aid and/or arranging for emergency medical care (including hospitalization) at my/our expense

2. Release 

I/we release MJMS from all claims and liabilities whatsoever arising from participation in or attendance at MJMS camps by myself/us, my/our child(ren) or any associated spectator(s)

3. Make-up Policy

I/we accept that there will be no make-up dates or refunds/credits offered for any missed days of the camp due to illness or other personal reasons

4. Withdrawal

I/we accept the MJMS camp withdrawal/cancellation policy, as follows:

Campers who withdraw more than 4 weeks prior to the start date of the camp week for which they are registered will be issued a full refund or studio credit, minus a $50 administration fee.

Withdrawals within 4 weeks of and up to 48 hours prior to the start date of the camp week for which they are registered are eligible for a 50% refund or studio credit, minus a $50 administration fee.

From 48 hours prior to the start of your child’s camp week onward, there are no refunds or credits issued for cancellations.

The above policy also applies to extended care.

Camper absence does not constitute a withdrawal.

Withdrawal requests must be done by email: weare@mjms.school 

Transfer requests will be considered up to 5 business days prior to the start of your child’s camp week. Transfer requests will be considered based on availability and are subject to a $50 administration fee.

5. Camper Absence

I/we accept that MJMS is unable to provide refunds or credits for camper absences of any sort.

6. Program Cancellation

I/we acknowledge that if an insufficient number of students have registered for a particular MJMS camp, or if required to do so in the interest of public health, MJMS reserves the right to cancel said camp. 

7. Camp Video & Social Media

One of the components of our camp is a music video, which the kids film to go along with their original song recording. We post the video on our YouTube page (which currently has 84 subscribers) and on our social media accounts. Our following consists almost entirely of MJMS parents and family members, but it is still public.

We don’t do any paid advertising for the studio and no child would ever appear in that kind of publicity campaign, but we understand that some parents are not comfortable with their kids appearing in any media that might be seen publicly, and that is of course absolutely fine. 

If your preference is to have your camper not appear in the video, they can still participate in all of the other aspects of the camp.

I/we give permission for my/our child(ren) to appear in the MJMS camp music video.
